1. AS SERVICES ARE RENDERED - Payment with cash or check at time of visit.
2. CREDIT CARDS - We accept VISA, MasterCard, Discover or American Express as payment for services as they are rendered.

Unicorn typically gives larger lines of credit for dental needs. Six and twelve month loans @ 0% interest. at these fees. 24 month loan terms require a 10% increase in fees.

INSURANCE POLICY

We require your estimated patient portion be paid for at the time services are rendered. We will gladly accept insurance assignment and we will process your insurance claim forms for you.
